I bet reloads are to blame for most of these Ar platform is pretty tough. Slam fires are really dangerous in semi auto rifles.

Why people fail to take notice of the lack of report or recoil when shooting is beyond me. My father taught me that if you fail to notice something feels wrong or sounds wrong, or notice it and ignore it....You have no business owning or shooting a firearm!

I was 8 years when he taught me that, I'll be 51 in December and I still remember and practice that lesson.

When I take someone shooting I brief all the what-ifs and watch over them, listening to the shots and watching the recoil. That way I can step in and prevent something like this from occurring.

When my Remington 597 had an out of battery detonation it just sounded and felt wrong. You have to listen and feel the gun to make sure every is running right

I had a squib load with some 9mm largo. The bullet went half way down the bore and the gun chambered the next round"Astra 400" I noticed something was wrong quickly. That could have been a bad day if I hadn't been paying attention.

#5 is likely case head separation. Gotta inspect your brass. Also, many of those look like squibs or double charges. Gotta be careful with reloads, check and double check everything. I never had an issue with any of my reloads because you gotta be careful.
